JOB DUTIES:

The professional RN care Coordinator provides patient centered, specialized, evidence based nursing care across the continuum through an inter-professional approach to treatment, research, education and advocacy. Responsible for ensuring the Care Management requirements for PCMH are met. Duties include bridging the gap between inpatient and outpatient care for the medically complex patient. Contributes to the goals of the department by being accountable for the delivery of compassionate and safe care within the scope of practice as defined by the Florida Board of Nursing and JHACH policy.

EXPERIENCE:

  1. Coordinate patient care across multiple settings, clarifying and discussing care plans with the healthcare team; provide care coordination updates to the provider and other healthcare team members ensuring continuity of care

  2. Maintain ongoing tracking and appropriate documentation of admissions and discharges, outpatient follow-up care visits and medication reconciliation to ensure providers have current and accurate information related to patient needs

  3. Meet with families to assess need for medical as well as community resources (food, housing, etc.); work with Referral Coordinator, Social Worker and other healthcare disciplines in order to best meet the needs of the family

  4. Participate in Quality Improvement processes to identify areas with opportunities for improvement

  5. Ability to collaborate with insurance case managers, social workers, care coordinators, and/or outside facilities to facilitate the care plan for the patient/family

  6. Follows organizational policies and procedures which support job responsibilities to work across multiple departments

MINIMUM QUALIFICATIONS:

• Current and active Florida RN license

• BSN

EDUCATION:

• 3 years' experience with Care Coordination

• BLS certification, AHA
