Offer customer and patient focused care that exceeds expectations at all times. This position provides for seamless continuity of nursing care between the regular business hours and after hours. This care may be in the absence of or as an adjunct to the RN Case Manager. Visits are made to hospice patients at home, facility, and hospital locations to manage emergent symptoms. Provide support to caregivers at the time of death. Demonstrate personal accountability for own continuous growth and development. Seek out appropriate opportunities to enhance competency and improve performance.
Caring Circle Hospice at Home is a community based non-profit organization serving 3 counties in Southwest Michigan: Cass, Berrien and Van Buren for over 30 years. We specialize in providing end of life care and symptom management to individuals with a life expectancy of six month or less. We are one of the few Hospice’s in Southwest Michigan with a 16 bed inpatient hospice residence to serve patients and families needing this environment. We serve individuals in their home, nursing facilities, and in the hospital. Team members enjoy how rewarding their job is, feel a strong sense of satisfaction while appreciating the interdisciplinary team approach to caring for patients and their families. They also experience healthier work-life balance with the flexibility that comes with hospice care for patients in their homes.
· Associate’s Degree in Nursing Required
· Bachelor’s Degree in Nursing Preferred
· 2 years of relevant experience in direct patient care Required
· Registered Nurse State of MI Required
