Job Summary:

The Nurse Care Manager works in conjunction with the CPRC team to provide education and support to clients who have or are at-risk for complex medical conditions in addition to mental health and or Substance-use disorders.

Key Responsibilities:

  • Develop wellness and prevention initiatives.

  • Facilitate health education groups by utilizing advanced knowledge of complex medical conditions.

  • Participate in the initial treatment plan development for all of their Healthcare Home enrollees.

  • Collaboratively develop a treatment plan of healthcare goals for individuals with co-occurring chronic diseases.

  • Consult Community Support Specialists regarding patient identified health conditions.

  • Manage the process of hospital and medical provider admissions and discharges for Healthcare Home patients.

  • Train patients on complex health conditions regarding medical diagnosis, treatment, and medications.

  • Implement, as required, DMH Net Health technology programs and initiatives such as Cyber-Access and metabolic screening.

  • Monitor and report performance measures and outcomes using sophisticated information systems developed for nurses.

    Education and/or Experience Qualifications:

  • Active RN or LPN license

  • Experience in direct patient care preferred

  • Knowledgeable in chronic disease processes in children and adults

  • Excellent written and verbal communication skills.

  • Ability to implement new innovative systems.

  • Ability to make decisions based on critical thinking and nursing process
