Summary This Senior Social Worker works with in the Health Care for Homeless Veterans Program of the VA Iliana Health Care System in the Peoria and surrounding communities. HUD-VASH is an intensive case management program for chronically homeless Veterans and the most vulnerable Veterans who have a history of homelessness and have complex health care needs. Duties Include: Provides case management for a caseload of Veterans in the HUD-VASH program, provides clinical case consultation on high-risk Veterans with colleagues in the VA and community-based programs, and coordinating prioritized admissions to permanent supportive housing programs in the VA and in through the community. Works collaboratively with local homeless response systems to ensure continuity of care among the VA and community providers. Attend By Name List meetings, manage interest list, and support the coordination of admissions to the VASH program. Support increased access to ancillary and supportive services for Veterans who are aging in place, including but not limited to transportation and referrals to supportive/assisted living programming. Support data reconciliation with housing authorities and case managers to ensure the accuracy of HOMES data and scorecard measures. Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to VA health care facilities, care in the community sites, VBA, VSOs, community resources, and coordinated entry systems. Participates in the monthly VISN 12 and National HUD-VASH Program conference calls and attends trainings offered by the National Homeless Program calls. Works with a broad spectrum of VA and community-based contacts to strive for developing more effective and efficient ways of delivery health care service to HUD-VASH Veterans. Performs accountability functions: 1) verifies Veteran's status, eligibility for VA health care and VA Homeless programming for service coordination needs; 2) document all supportive services and case management contacts with Veteran/families/ancillary supports in CPRS; 3) administrative support for program coordination, development and implementation of written procedures specific to the community and surrounding areas, and complete reports Work Schedule: Monday - Friday; 7:30am - 4:00pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Education: Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure: Must be licensed or certified by a state at the advanced practice level at the GS-12 grade level. This includes an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (PROVIDE COPY) IN ADDITION TO THE BASIC REQUIREMENTS YOU MUST MEET THE FOLLOWING GS-12 GRADE REQUIREMENTS: GS-12 Grade level: EXPERIENCE: must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. AND Applicants must also meet the Demonstrated Knowledge, Skills, and Abilities listed below: 1. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. 2. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. 3. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. 4. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. 5. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. *A valid, unrestricted state Driver's License is required.
