We are looking for per diem LVNs to work alongside our RN case managers to help deliver exceptional clinical care with a personal touch. Responsibilities: * In this job, you will be responsible for conducting working with an RN to treat and educate patients according to the established care plan. 1 year experience as an LVN is required, home health and/or Wellsky (Formerly Kinnser) EMR experience is a plus! Qualifications: * The ideal candidate for this role will have the following qualifications: Be a graduate of an accredited School of Nursing, have a current TX license to practice as an LVN, be comfortable being independent in the home setting with skills such as assessment, venipuncture, wound vac, and Foley catheterization, have current CPR certification, and have reliable transportation with current auto liability insurance.
