Description Clinical Pharmacist - Transitions of Care at Providence Sacred Heart Medical Center - Spokane, WA This position is a dedicated transitions of care pharmacist primarily focused on admission medication reconciliation for our hospitalized patients. Potential for leadership growth as this position oversees technicians and routinely precepts students and residents. Required qualifications: PharmD Doctorate of Pharmacy Or Bachelor's Degree in Pharmacy Upon hire: Washington Pharmacist License when eligible and where applicable, obtain a Board of Pharmacy Preceptor license Where applicable Basic Life Support-Provider (BLS) Preferred qualifications: Acute care hospital pharmacy experience, 1 year or equivalent experience Why Join Providence?
