Summary:

Provides statewide clinical nursing and procedural expertise by telephone and video for Northern Light Home Care & Hospice patients, families, facilities, physicians, vendors, and staff. While managing on-call accountability for both home care and hospice, the incumbent is specially trained in the hospice concept to provide on-call services for the clinical, personal, physical, and emotional needs of the patient and family via the telephone and videoconferencing. The Registered Nurse- Triage effectively coordinates the appropriate care of patients/families during the evening/night hours by collaboration and communication with the on-call team as appropriate. The Registered Nurse- Triage is responsible for communicating with the entire care team to make them aware of any issues or needs arising during the triage shift, via email, video conferencing and/or verbal communication needed.

Responsibilities:

  • ASSESSMENT AND NURSING INTERVENTION VIA TELEPHONE TRIAGE: Consistently performs skilled nursing assessments via telephone for home care and hospice patients, using video visits as appropriate; instructs patient and family members on interventions for care in accordance with the patient’s plan of care; develops appropriate plans of action to resolve after-hours problems including assessing for appropriate level of care, initiating change in level of care as needed; provides clinical expertise to nurses and other clinical staff as issues arise in the field; coordinates patient visits with local field clinicians to ensure the needs of patients and families are addressed in a timely manner.

  • PROVISION OF CARE: Responds to inbound telephone calls, voicemails, and emails in a timely manner and triages calls to determine appropriate action needed to address issue; arranges for medications and/or supplies needed during evening or night as appropriate, which may include obtaining physician orders; arranges with local on-call field clinician for a patient visit if needed. Makes tuck-in calls to newly admitted patients (as time allows) and to any patient who has experienced any change or problem as requested by a team member; performs EMR audits/reviews as time allows.

  • COORDINATION OF CARE THROUGH COMMUNICATION AND COLLABORATION: Completes thorough documentation in real time, including clear documentation on issues, interventions, outcomes/effectiveness, and MD orders; accurately report problems/needs/changes via email/phone call to the patient’s team the which occurred during the night

  • SELF-DIRECTED, RESPONSIBLE, AND PROFESSIONAL BEHAVIOR: Has a thorough understanding of the practices in accordance with Northern Light Home Care & Hospice policies and procedures. Actively involved in problem solving and quality improvement regarding any identified after hours issues/concerns/trends; demonstrates ability to solve problems independently while seeking consultation when needed.
