Responsibilities:

  • Formulates patient care plans with daily goals, considering the patient's individual needs and involving them as partners.

  • Communicates patient conditions and ensures comprehensive documentation that enhances caregiver communication.

  • Prioritizes patient safety, quality, and satisfaction by incorporating professional standards and health system goals.

  • Builds caring, compassionate relationships with patients, families, and colleagues, contributing to a positive patient experience.

  • Provides effective teaching to patients and families throughout care transitions.

  • Seeks performance feedback and engages in continuous professional improvement and organizational programs.

  • Identifies opportunities for clinical and workplace improvement, while maintaining strong internal and external relationships.

Licensure, Certifications, and Clearances:

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

  • Licensed Practical Nurse (LPN)

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
