JOB DESCRIPTION Job Number: 212138 Department: Care Management Services Hours: 8:00a - 4:30pIn this Inpatient Social Work position you will provide social work interventions for Medical Center patients and family needs.Coordinate discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agenciesPerform a psycho/social assessment for continuity of care planning for in/outpatientsProvide short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability. Qualifications:Master's of Social Work from an accredited college or university is required.Registration with the state of Washington as either a Licensed Social Work Associate (LSWAIC), Independent Clinical Social Worker (LICSW), Social Work Associate Advanced (LSWAA) or Social Work Advanced (LISW)May consider an Agency Affiliated Counselor with the expectation of active pursuit of their Social Work Licensure.Strong interpersonal and organizational skillsWorking knowledge of individual and family dynamics and the ability to perform psycho/social assessmentsUnderstanding of systems and how to work within systemsUnderstanding of the impact and implications of disease and the course of diseaseVirginia Mason offers much more than competitive compensation and benefits.
