Physical Therapist - PT - Outpatient PACE Program

MORE ABOUT THEPT PHYSICAL THERAPISTPOSITION:

CareerStaff Unlimited partners with companies around the country to raise the bar in healthcare by setting new standards for quality care and personalized service. Responsibilities for thisPhysical Therapistopportunity include, but are not limited to:

  • Evaluates participants, administers physical therapy treatments, records progress and instructs participants and/or family in care as part of Healthy Aging and Wellness Program.

  • Supervises work activity of assistant(s).

  • Our community health based model allows our therapists to work in the PACE outpatient center and in participants’ homes as needed.

  • Conducts initial assessments and periodic reassessments.

  • Develops and routinely updates plans of care, regularly informing the interdisciplinary team of the medical, functional, and psychosocial condition of each participant. Remains alert to pertinent input from other team members, participant caregivers.

  • Documents changes in a participant’s medical record consistent with documentation policies.

JOB REQUIREMENTS FOR THE PT PHYSICAL THERAPIST:

  • Masters of Science or Doctorate in Physical Therapy from an accredited program

  • Successful completion of National Certification Examination

  • 1 year of verifiable, supervised professional experience within the last 3 years

  • Valid PT license in the state of practice
