Swedish Health Services Patient Safety Officer - Risk Management in Seattle, Washington
Swedish Medical Center is looking for a Patient Safety Officer (1.0 FTE, Day Shift) to work in Risk Management at Swedish Shared Services in Seattle, WA.
Responsible for patient safety with primary responsibilities related to support of the Swedish patient safety team as well as support of the Culture of Safety Initiative. The patient safety role includes being a member of Swedish organizational response to quality variances and having or developing skills to evaluate variance reports, perform root cause analysis and Healthcare Failure Mode Effect Analysis and investigations for serious safety events in support of the patient safety program. Participate and lead quality and safety meetings throughout the organization.
In this position you will have the following responsibilities:
Identify high-risk areas through Root Cause Analysis (RCA), case investigations, e-QVR referrals, phone referrals, incidents/accidents, and other relevant data sources.
Conduct Root Cause Analysis (RCA) to identify “root causes,” risk reduction strategies, and improvement strategies.
Conduct interviews with physicians, independent practitioners, managers, clinical nurse specialists, and staff in order to identify case specific details and to develop a complete understanding of events.
Develop support systems for staff and physicians involved in unanticipated events.
Review records, researches cases, and create formal documentation of adverse events and sentinel events.
Required qualifications for this position include:
Bachelor's Degree in Nursing Baccalaureate degree (BSN) from an accredited school of nursing or must actively pursue and attain a BSN within six years of hire
2 years clinical experience
Project management experience
Experience in change management with internal & external customers including physicians, staff, managers, and directors
Current license to practice as a Registered Nurse in the State of Washington
Preferred qualifications for this position include:
Master's Degree in Nursing, Business, Education, Hospital Administration or equivalent educ/experience
Process improvement experience
