HARBORVIEW’S PEDIATRICS’ – YOUTH CLINIC Department has an outstanding opportunity for a MENTAL HEALTH PRACTITIONER / CASE MANAGER WORK SCHEDULE

• 60% FTE, 24 hours per week

• Day Shift POSITION HIGHLIGHTS

• A critical member of the Department of Pediatrics’ Youth Clinics for homeless and unstably housed youth

• Work collaboratively with the Youth Clinic team, site personnel and other homeless health care providers DEPARTMENT DESCRIPTION MHP is responsible for interacting with the patient population at the sites served. In addition to a high level of clinical skill & competence, the MHP will be an excellent communicator with patients, shelter staff, and other medical providers. The MHP will function within the scope of licensure and in accordance with department, hospital, and university policies/standards.

  JOB RESPONSIBILITIES

• Participate as a member of a multi-disciplinary team to serve homeless youth 12-26 years old, some of whom are using substances and/or have a mental illness

• Conduct psychosocial, substance abuse, and social service needs assessments

• Develop treatment plans, provision of case management, and psychotherapy services

• Make referrals to other services or agencies, and assists clients in following through with those referrals

• Work with a child/adolescent psychiatrist, may recommend and monitor use of medication in mental health treatment

• Participate in psychiatric consultation, supervision, program meetings

• Acts as an advocate for the patient through such activities as organizing patient care conferences and safeguarding patient’s privacy and confidentiality REQUIREMENTS

• A Master’s Degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work

• OR Bachelor’s Degree in nursing or a behavioral science AND three years’ of experience in the direct delivery of professional mental health care

• Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration
