Job Summary

The Y is seeking qualified and dedicated individual for the position of WISe Coach and Lead Care Coordinator. You will provide leadership and support to the Wraparound with Intensive Services (WISe) Team and ensure that all clients in the WISe Program are receiving quality wraparound services. You will also provide program coordination activities, including coordination of referrals and services, as well as some administrative, operational, and program development support. As assigned, you may also provide care coordination services to a caseload of clients enrolled in the WISe Program. Care coordination services include coordinating or providing services to enable the youth and caregivers to utilize medical, educational, social, clinical, and other services necessary for maintaining the youth’s physical, psychological, and/or developmental health.

This position is with the Y Social Impact Center, the social services division of the YMCA of Greater Seattle (YGS), serving around 5000 youth, young adults, and families every year in communities across King County, Pierce County, and various counties across WA. Through our programs we aim to strengthen young people’s ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; establishing stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center’s programs include homelessness prevention, housing, behavioral health, crisis response, foster care licensing, violence prevention, and employment. Since many of the young people we serve have been involved in the foster care, criminal justice, behavioral health, and/or homelessness systems, we truly value relatable lived experience of our staff and those applying for our positions as an asset.

    Hiring Range: $27.00-$30.00/hour

Responsibilities

  • In coordination with teammates within the WISe program and the broader Foster Care Services division, ensure all required intake, service planning, and discharge/exit documentation is completed fully, on-time, and in accordance with contractual requirements, accreditation standards and best-practice.

  • As assigned, maintain a small caseload of clients, providing case management and care coordination services to clients enrolled in TAY WISe services, as outlined in the WISe contract and manual.

  • Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs in a timely and effective manner.

  • Ensure client cross-system care plans are developed to fully represent the client’s needs and strengths, are in accordance with WISe core values, and are reviewed regularly by WISe teams.

  • Assist in ensuring all WISe providers are trained to the fullest extent in WISe principles and practices. Undergo WISe Coaches training and other trainings specific to leadership in this treatment model.

  • Attend regular meetings within the King County WISe community including but not limited to the monthly WISe Collaborative meeting, the CRT, the FYSPRT, and the coaches support team.

  • Proactively engage housing services, contacts within juvenile rehabilitation, and other community resources to identify young people youth in need of wraparound services and assist in managing the interest list for the WISe program.

  • Ensure all WAC and contract required client case notes, progress reviews and reports are completed thoroughly and accurately, are well written, and submitted on-time. Ensure each client’s file/record is fully in compliance with WAC, DCYF, DOH, DSHS/DBHR, KC-BHO, MCO, COA Accreditation, and YMCA requirements.

  • Ensure all clients’ information is kept confidential and only released in accordance with HIPAA, DCYF, DSHS, MCO, JR, and YMCA policy and procedure.

  • Receive clinical supervision and participate in clinical consults for clients as necessary in order to be able to provide clinical advocacy for clients. In conjunction with WISe Program Director and mental health professional, assess clients’ mental health needs and determine appropriate level of care and support

  • In coordination with the Foster Care Services leadership team, recruit, screen, and interview new case managers. Facilitate pre-service and in-service training, and ongoing development of care coordinators specifically and WISe providers more broadly.

  • Serving in a lead role, provide ongoing regular support, leadership, and guidance to the WISe team. Facilitate and lead regular meetings/consult with the team to ensure that each member’s needs are being met and to promote teambuilding.

  • As assigned, represent YMCA in external meetings focused on policy, practice, and advocacy efforts related to child welfare/behavioral health services.

  • As assigned, participate in program, branch, and organizational leadership roles, including meetings & activities. Serve as a member of the branch leadership team.

  • Other duties as assigned.

    Position Type/Expected Hours of Work

    This is a full-time position, and hours of work and days are generally Monday through Friday, 8:30 a.m. to 5:00p.m, however flexibility is required with some evening and weekend work required as job duties demand.

Qualifications

  • Bachelor’s degree in psychology, social work, administration or related field or equivalent experience, Masters preferred. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.

  • Minimum three years of experience working with teens or young adults in foster care, group care, housing, education, juvenile justice, anti-poverty, vocational support, mental health, chemical dependency and/or homelessness fields.

  • Ability to teach effective independence skills for a variety of learning styles and to plan supportive activities.

  • Strong youth engagement and relationship-building skills.

  • Strong verbal and written communication skills, including skills in coaching and giving/receiving feedback.

  • Strong organizational skills.

  • Basic computer skills and working knowledge of Microsoft Office.

  • Ability to transport self and client(s) in a safe and effective manner, and to meet clients at community locations; proof of adequate vehicle insurance coverage is required, or ability to travel to areas without public transportation.

  • Driver’s license required.

  • Current state approved First Aid and CPR certification.*

  • Current approved HIV blood borne pathogens training.*

  • Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful.

    • Within 30 days of employment or first available training.

    Additional Preferred Qualifications:

  • Minimum one year of experience supervising employees or full-time interns, volunteers or national service members (AmeriCorps, etc.).

    Authorized Driver Requirement

    This safety-sensitive role requires transporting clients as “authorized drivers” for the Y using your vehicle. To meet this requirement, you must be age 21+, have a valid driver's license for at least three years and possessing a driving record that meets Y standards (no more than two moving violations within the last three years, no significant incidents such as reckless driving, DUI/DWI, no tickets 20+ miles above the speed limit etc. within 5 years) and proof of current auto insurance; minimum recommended coverage limits are $300,000 per incident, and current vehicle registration is required. You must also pass a drug screen that will test for impairing substances, including marijuana.
