Responsibilities Job Summary Provides patient related care tasks based on the principles of growth and development across the life span of patients served in the home enviroment. Assists, observes, and reports patient changes to Registered Nurse or other appropriate skilled professional and maintains accurate medical record documentation relative to the care delivered. Responsible for supply management to clinicians and patients. Provides and collaborates direct patient care services under the direction of a Registered Nurse or appropriate skilled professional. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and implementation of select portions of the patient care plan. Home Health aides must be members of the interdisciplinary team, must report changes in the patient’s condition to a Registered Nurse or other appropriate skilled professional and must complete appropriate records in compliance with the home health policies and procedures. Orders & maintains supplies for clinicians in the home environment and patients. Performs other duties as assigned. Qualifications Education High School Diploma or GED Required Completed Certified Nursing Assistant Training Required Licenses and Certifications Basic Life Support (BLS) Required Certified Nursing Assistant (CNA) Required Fingerprint Clearance Card - State Current Arizona Dept. of Health Services Fingerprint Clearance Card or shall apply and be eligible for a fingerprint clearance card within twenty (20) working days of employment Required Driver License Required Auto insurance Required
