We are seeking experienced home health professionals who enjoy variety, mobility and are focused on the needs of our patients.:: Position Summary: We are currently looking to add to our Clinical Management team to support our growing census! The Clinical Supervisor assists the Director of Nursing with the supervision of home health services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. Nursing services furnished are under the supervision of the Clinical Supervisor. The Clinical Supervisor is available at all times during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel. Standard schedule is Monday-Friday with possible On Call rotation for eves weekends. This position is based out of the Sante’ Operations office (Princess 101) in North Scottsdale. Essential Job Functions, Responsibilities and Activities: * Participates in the development of standards which ensure safe and effective service to patient/family. * Ability to supervise, guide and develop nursing skills and performances. * Consults with the Director of Nursing in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of health personnel. * Assists in recruiting, hiring, evaluating and terminating nursing personnel. Assists in instituting, interpreting and documenting disciplinary action. * Responsible for the orientation of new patient care personnel. * Assists in the planning and consultative needs of associates. * Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, state and local laws. * Displays a willingness to support policies and procedures and uses appropriate channels for changes of such policies. * Assists in the coordination of interdisciplinary team services. * Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service. * Participates in the coordination of Agency’s services with the services of other community agencies. * Participates in studies and research and other administrative functions as assigned. * Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performances. * Serves as a resource person to associates. * Develops a cooperative relationship and communicates effectively and professionally with physicians. * Investigates and reports any problem relating to patient care and/or employee well-being. * Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Nursing. * Attends pertinent continuing education programs other than routine in-services and shares information with associates. * Monitors equipment for appropriate use and takes steps to keep misuse to a minimum. * Assists with the Agency’s ongoing performance improvement (PI) program. * Assists with the development, implementation and evaluation of patient plans of care. * Reviews patient medical records for compliance with federal, state, local and Agency policies and guidelines. * Assists Director of Nursing in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects. * Ensures that all necessary supplies and equipment are available. * Must be able to perform nursing functions in times of staff shortage. * Available to share on-call with nursing personnel as needed. * Ensures that documentation is completed in accordance with Agency policy. * Assists with staffing visit assignments and on-call. * Other duties as assigned by the Director of Nursing. * Acts as Director of Nursing in the Director’s absence. Required Experience, Qualifications, Knowledge, Skills and Abilities: Education: * Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program. Licensure and/or Certification: * Prior to date of hire, must hold a current, valid RN license or temporary RN license through the Arizona State Board of Nursing, or hold a compact RN license. Must maintain valid licensure to practice as a RN. * Valid driver’s license with maintenance of safe driving record and an automobile that is insured in accordance with state requirements. * CPR Certification and Fingerprint clearance card required. Experience: * Minimum three (3) of experience in nursing, required. * Minimum one (1) year experience in home health nursing supervision, required. * Experience with WellSky (aka Kinnser), OASIS-D and able to train skilled nursing duties as needed. Skills, Knowledge, and Abilities: * Ability to exercise initiative and independent judgment. * Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
