Responsibilities Job Summary This position is responsible and accountable for incorporating the mission, vision, values, and critical goals of the company's development and operations Care Management Department. In addition, the CCO will provide innovative Care Management (both Transition and Ambulatory Care Management) design, development and operations at both a system and operations level, to ensure consistent, high quality provision of care coordination, transitional planning and utilization management considering the unique needs of each patient, the preferences of the providers and the requirements of each of the payers. Position develops and oversees the ICP Utilization Management and Prior Authorization department. Develops, leads and directs the implementation of efficient and effective care management programs for patients. These programs will span the continuum of care, utilizing care management principals of care coordination, transition planning, utilization and disease management. Ensures compliance with IOCP minimum requirements where applicable. Manages the UM/PA team including quality improvement programs. Evaluates and fosters collaborative partnerships within the inpatient and outpatient setting and outside relationships. Develops and maintains community relationships, including other patient care facilities, agencies, other hospitals, health plans and vendors. Works closely with the CEO, CMO, CSO, Hospital Case Management AVP and Directors and others to optimize the coordination of care provided to patients. Participates, develops, and implements strategic metrics, monitoring protocols and short and long range utilization goals to achieve the necessary clinical and financial outcome measures Assures the implementation, achievement and evaluation of the utilization goals and action plan of the system. Hires, directs, manages, mentors and evaluates Care Management personnel assuring consistent monitoring and implementation of staff competencies and budget impact. Sets directions and provides oversight for recruitment and retention strategies. Promotes departmental climate of interdisciplinary teamwork through the utilization of the strengths of all disciplines, inter-department collaboration, win-win decision-making, personal accountability and employee satisfaction. Develops, implements and maintains Care Management programs, policies, space utilization, annual capital and operational budgets for the department Responsible for writing/creating policies, procedures, guidelines and reports for the Care Management team. Monitors compliance with all regulatory requirements. Performs other duties as assigned. Qualifications Education Bachelor's Degree in Nursing, Health, Administration/Management, MPH or related field or completion of a PA Program Required Experience 3 years managing a Care Management Department Required Licenses and Certifications Nursing\RN - Registered Nurse - State Licensure And/Or Compact State Licensure Nursing\RN\PA - Registered Nurse or Physician Assistant (Valid/Current) - State Licensure And/Or Compact State Licensure Required
