Job Summary and Qualifications

This position plays a key role in ensuring safe and effective transfers in the movement of patients across the care continuum, serving as the bridge between the professional staff in the hospital, and the patient and/or family. In this role, you will provide information and guidance to the patient and/or family for an effective care transition, improved self-management skills and enhanced patient-practitioner communication. In addition, the High Risk Perinatal Care Coordinator will also work directly with the Perinatal Outreach Coordinator.

Complies with the Memorial Health Compliance Program, related policies and procedures and the Code of Business Practices; reports instances of perceived and actual non-compliance to Team Leader or through other reporting mechanisms.

Standards of Practice

1.Assessment: Collects comprehensive patient/client health data and plans teaching opportunities for patients or healthcare providers

2.Diagnosis: Critically analyzes the assessment data in determining the diagnosis, problems, issues or barriers to care.

3.Outcome Identification: Identifies expected outcomes deemed from the assessment data and diagnosis and individualizes expected outcomes with the client and with the health care team, where appropriate.

4.Planning: Develops a comprehensive plan of care that includes interventions, coordination of treatments, and links patients to appropriate resources to attain expected outcomes/educates staff of plan.

5.Implementation: Implements a navigator plan of care for the patient/client. Provides education to patient/family/nursing staff regarding diagnosis, plan of care, and identifies needs and barriers to care.

a.Care Management/Coordination of Care: provides comprehensive clinical coordination of care and care management.

b.Consultation: provides consultation to influence the plan of care for clients, enhances the abilities of providers and others and effect change in the system for improved patient outcomes.

c.Health Promotion, Health Maintenance and Health Teaching: employs complex strategies, intervention and teaching to promote, maintain and improve health and prevent illness and injury.

d.Assists patients to obtain a PCP and follow-up appointment

  1. Evaluation: Evaluates the patient/clients’ progress in attaining expected outcomes, on individual basis and discusses with nursing and physician staff.

What qualifications you will need:

  • Bachelors Degree

  • Three years’ clinical experience with perinatal patients in a hospital setting. Perinatal nursing care preferred.
