Dignity Health Medical Group – Dominican (http://dhmf.org/dominican) ,, is seeking an energetic and motivated Advanced Practice Provider to join our collegial practice in Santa Cruz, CA . Compensation/Benefits Include:
Compensation Range: 146,507 - $164,088 DOE
Productivity bonus, sign on bonus and relocation assistance
Comprehensive benefits including health insurance, retirement plans and CME allowance
Responsibilities
Patient Assessment and Diagnosis:
Conduct comprehensive health assessments.
Diagnose and treat acute and chronic illnesses.
Order and interpret diagnostic tests.
Treatment Planning and Management:
Develop and implement patient care plans.
Prescribe medications and therapies.
Provide health promotion and disease prevention education.
Collaboration and Consultation:
Collaborate with other healthcare professionals.
Consult with physicians and specialists as needed.
Coordinate patient care across various settings.
Qualifications
A valid and unrestricted license to practice in the State of California
Nationally recognized certification (as determined by Medicare)
Completion of state mandated educational courses
Pay Range
$70.43 - $78.88 /hour
