OUTREACH MISSION INC. Employee Job Description Street Outreach Case Manager Job Title: Street Outreach Case Manager Department: Client Services Shift(s): Hourly 8:00 am - 5:00 pm Monday through Friday or as needed to support clients Immediate Supervisor: Executive Director Outreach Mission Inc. (OMI) participates in a Housing First philosophy. Housing First is a homeless assistance approach that prioritizes providing permanent housing to people experiencing homelessness, thus ending their homelessness, and serving as a platform from which they can pursue personal goals and improve their quality of life. This approach is guided by the belief that people need necessities like food and a place to live before attending to anything less, such as getting a job, budgeting properly, or attending to substance use issues. Additionally, Housing First is based on the theory that client choice is valuable in housing selection and supportive service participation, and that exercising that choice is likely to make a client more successful in remaining housed and improving their life. General Job Description: Provide street outreach services for homeless men, women, and women with children, utilize program and resources and formulate plans that promote moving towards self-sufficiency including shelter, food, clothing, mental health, substance use, medical health, and employment/income strategies. Ensure the safety of all clients by monitoring and enforcing the rules, policies and procedures for street outreach engagement. Essential Job Functions: Function 1. Client Engagement Process: Follow policies and procedures for comprehensive engagement of clients to identify individual client needs. Elements include but are not limited to: 1) Meet clients where they are. Utilize local homeless community maps and daily location information to plan client engagements. 2) Effectively conduct client interviews in an informal manner to minimize communication and relationship barriers. 3) Act upon critical client needs appropriately and immediately. 4) Orient client to available agency benefits and benefit locations. 5) Utilize approved HMIS demographic information forms and information release forms for each client desiring services. 6) Establish and monitor appropriate client confidentiality procedures. Case Management: Develop a client-driven case plan for street outreach clients with both short-term and potential long-term goals identified. This may require multiple client engagements to build trust and establish open communications and plan development in stages. 1) Continually look for opportunities to get client sheltered. 2) Assist client with resources to fulfill case management plan (e.g. transportation strategies, medical needs, substance use needs, job coaching, accountability conversations). 3) Ensure that a written case plan utilizing HMIS is completed for each client. 4) Complete a timeline and measures for each goal identified. 5) Monitor progress towards goals in regularly scheduled engagements. 6) Conduct client engagements to coach plan elements, make referrals for needs; document client progress. 7) Assist Executive Director with identifying street outreach program needs and potential grant funding. Assist in grant writing as necessary. 8) Maintain a list of client stories for senior leadership, fundraising and marketing staff. Function 3. Record Keeping and Reporting: 1) Maintain client files in HMIS and/or client files to include conversations, warnings, progress towards goals and documentation of any incidents. 2) Collect data as necessary to meet funding requirements and statistical reports. 3) Maintain NC HMIS to meet all state, federal, and grant requirements. Function 4. Special Projects: 1) Develop, lead and participate in OMI special project work as assigned. 2) Participate in Continuum of Care and community groups as assigned. Function 5.
