Job Summary and Qualifications

This positionacts as a liaison between internal and external referral sources, and acute inpatient rehab operations. The Clinical Rehabilitation Specialistis responsible forestablishingrelationships with case managers, physicians, payers, and other referral sources. The Clinical Rehabilitation Specialist will ensureappropriate patientplacement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director.

Job Duties and Responsibilities:

  • Assistand coordinate in-take and pre-admission screening process. Accurately documents evidence of rehab admission criteria on Pre-Screen evaluation ina timelyand legible manner. Pre-screen updates are completed within48 hoursof admission. Admission decisions are communicated to referral source within 2 hours.

  • Provides information related to inpatient rehab and program features to patient and family members.

  • Communicates to patient’s expectations and requirements for participation upon admission to the rehab program. Offers each patient and family a tour of the rehab unit; coordinating tour if unable to provide personally.

  • Secures information relating topatientsresources and benefits.

  • Demonstrates clinical knowledge of rehab admission criteria, leading toappropriate identificationof qualified rehab patients.

  • Advisesreferral sources on inpatient rehab criteria.

  • Functions as primary contact for external and internal case management and referring physician. Establishes andmaintainseffective working relationships with all referral sources.

  • Completes face-to-face, in-service educational contacts with physicians, case managers, other interdisciplinarymembersand other professionals. Serves as educational resource to patients, family members and caregivers.

  • Participates in market analysis, data analysis and development and implementation of marketing and business plans for the Rehab Unit. Contributes to the development and maintenance of referral source information such as brochures, flyers, referrals and information kit and other outreach and marketing tools.

  • Proficient in time management. Prioritizes meeting commitments to referral sources. Develops a call-plan to prioritizeutilizationof time.

  • Assistsin developing the annual strategic plan for the Rehab Unit/Hospital, to include visible community outreach program and raise community awareness of services available.

  • Ensures referral sources receiveappropriateandtimelyfollow-up communicationsregardingtheir patients and hospital programs.

  • Develops back up system to handle calls when he/she is unavailable.

  • Develops system that ensures calls are appropriately handled 24 hours a day, 7 days a week.

  • Reliable transportation to visit referral locations.

  • Assistas needed with PAI transmissions.

  • Other duties as assigned.

  • Perform on-site and off-site clinical evaluation toassistRehab Medical Director indeterminingappropriateness of patient for admission.

Qualifications:

  • Licensed Professional in a clinical health care specialty (RN, Physical Therapist, Occupational Therapist, or Speech Therapist). Current FL licensure or FL approved compact license.

  • American Heart Association or Red Cross BLS

  • Current state licensure in clinical practice area or eligible for state license transfer.

  • Graduate of an accredited school in clinical specialty.

  • Minimum 2 years recent ACUTE Rehab clinical care experience.

  • Skills,Knowledgeand Abilities:

  • Participates in weekend rotation schedule to ensure continuous access by referral sources to rehab services for evaluation and admission to program 7 days a week.

  • Computer and keyboarding skills (25 wpm minimum).

  • Complies withall regulatory requirements including all State, Federal and Joint Commission regulations.

  • Ensures confidentiality.

  • Representsthe facility in a positive and professional manner.

  • Ability to work with others to ensure quality patient care.

  • Supports the mission and vision ofthe allHCA facilities in the Market. Understands and is committed to helping HCA facilities achieve its goals.

  • Must learn and comply with System and facility safety policies and rules; must use appropriate safety equipment and procedures at all times; must immediately report all unsafe conditions to supervisors; must be familiar with all safety features of equipment, tools or materials encompassed by job duties; and must check with supervisors (prior to job performance) if there is a question as to the safe procedure to be used for any job function.

HCA Florida Lake Monroe Hospital is a 221-bed acute care hospital. We serve the communities of Seminole and West Volusia County.
