In this role, you’ll make a difference in the lives of people with substance abuse challenges, and often with co-occurring conditions. You’ll use and grow your skills in compassionate care, crisis intervention and de-escalation, in a supportive and inclusive work environment. You’ll have the opportunity provide our community with substance abuse treatment to help people who are just discovering possibilities in their journey, going from a place of hopelessness to a place where they are valued and have the potential for a better life.

The case manager is responsible for case management and some referral planning for clients in the Horizon Services, substance use detoxification center. Duties include care coordination, maintaining positive working relationships with outside community providers, scheduling appointments, arranging transportation, assisting with acquiring medications, ongoing care and discharge planning, and crisis intervention. The case manager will collaborate frequently with Program and Personnel Manager, Detox Program Counselor and Medical Staff (referring to LVN, EMT) to ensure clients basic needs are being met in preparation for stabilization and discharge planning

What will make you successful in this role?

  • Provide case management services to clients residing at the Detoxification Center.

  • Assist Detoxification Department staff with coordinating all client withdrawal management needs while clients are residing at Palm.

  • Schedule and arrange client medical, psychiatric, and or continuing care appointments.

  • Provide ASAM screening and referral assessments as needed.

  • Maintain appropriate notation and documentation in Electronic Health Record system. 

  • Maintain accurate documentation in accordance with Case Management billing practices.

  • Provide timely crisis intervention when needed.

  • Meet daily with clients to determine case management and referral needs.

  • Participate in all required consultation with San Mateo County regarding ASAM and other requirements

  • Maintain positive working relationships with community providers and managed care organizations.

  • Assist with case management training at monthly staff meetings.

  • Provide clients with educational materials and orientation of continuing care options, including residential treatment and recovery residences.

  • Demonstrate good decision making on a daily basis.

  • Utilize email and other forms of technology to communicate with peers, supervisors, and community members.

  • Assist with data entry in to EHR system as needed.

  • Maintain a basic knowledge of medications used by clients.

  • Drive program vehicle when necessary.

  • Perform any other duties as assigned by Program and Personnel Manager.

    Technical Training or Knowledge

  • Knowledge of alcohol and drug addiction

  • Excellent oral and written communication skills

  • Excellent computer skills, knowledge of data capture and data input into Electronic Health Records.

  • Ability to communicate using email and other electronic systems.

  • Familiarity with Substance Use programs, hospitals, and other continuing care options

     

  • Experience

  • Experience working with customers or clients that feel overwhelmed or in a crisis (crisis intervention).

  • Experience working with clients who have Substance Use disorders.

  • Extensive experience with case management of underserved populations in a public setting.

  • Experience working with diverse populations and cultures.

  • Experience working with customers or clients with co-occurring disorders and disabilities.

  • Familiar with customer service principles.

  • Experience with resolution of conflict in stressful situations.

    What else is required?

    Must be registered to certified with a State AOD certifying body such as CCAPP, CADTP or CAADE

  • Valid California driver’s license with a good driving record and car insurance.

  • Current CPR/First Aid certificate desirable .

  • Drug testing required at time of hire.

  • The California Department of Public Health has required that all staff in healthcare organization receive the COVID 19 vaccination and booster, our organization is a healthcare organization and will abide by this mandate.

    Salary:  $28.00-$30.00/hr based on experience and qualifications. Available for a full-time position.
