University of Utah PS Nurse Practitioner in SALT LAKE CITY, Utah
The NP/ APRN or Physician Assistant role provides care and treatment to patients in the University of Utah Health Inpatient Geriatric Consult Service and ambulatory Geriatric Patient-Centered Medical Home. This position will be working as part of a team with a geriatrician attending and fellows to care for patients 65 years and older.The divisions clinical practitioners provide state of the art care for older patients in outpatient clinics, inpatient and hospital programs, and long-term care settings. The care provided addresses each of the 4 Geriatric Ms, guiding our participation in the Age-Friendly Health System initiative. The division currently has thriving team-based care programs in the areas of Geriatric care, including its Patient-Centered Medical Home; dementia and other cognitive disorders in its Aging Brain Care program; and mobility in its Faint and Fall clinic, the in-patient delirium prevention program, the Hospital Elder Life Program (HELP), and an in-patient consultative service.This position may be required to access and administer medications within their scope of practice and according to State Law.
