Peninsula Regional Medical Center RN - Patient Care Coordinator in Salisbury, Maryland
The primary function of the RN patient care coordinator is to coordinate the patients care across all departments, disciplines and providers. They ensure appropriate continued stay utilization, as well as clinical information to outside review agencies and while provide appropriate and safe discharge planning.
Bachelor’s degree preferred and current MD RN licensure required.
Case Management Certification preferred.
Minimum of 3 to 5 years experience in an acute care hospital.
Home health, case management experience with an insurance carrier or HMO and/or Utilization Review and discharge planning experience preferred.
Strong interpersonal and organizational skills.
Strong verbal and written communication skills.
Working knowledge of compute programs
Demonstrated skill in working as part of a team.
Demonstrated ability to prioritize multiple tasks and manage time effectively.
Ability to work independently and exercise sound judgement in interactions with payors, providers, patients, and families.

Contributes to department budget development and efficient use of department resources.
Contact with others
- High level of contact with patients, staff, administration, management and public.
- Day shift Monday-Friday, Evening and weekend work as assigned.
