Community Health Worker

Location: HOAP Day Center | 694 Church St. NE, Salem OR 97301

Job Status: 40 hours per week, 8:30am - 5pm, Monday - Friday

Sign-on Bonus: $1000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)

Employee Referral: $500 (Referral bonus’ expire at the close of this job posting.)

YOUR ROLE:

We are looking for someone passionate about increasing access to healthcare among the houseless population. This person is excited to help clients & patients overcome barriers in reaching their health, housing, and employment goals. As a Community Health Worker (CHW), you will develop a deep understanding of social determinants of health and their effect on health inequalities and serve as the link for individuals between health care services and community-based organizations (CBO’s). CHW’s are patient advocates, approachable, critical thinkers, dependable, continual learners, persistent, organized, practice cultural humility, demonstrate a can-do attitude, and take initiative.

As a Community Health Worker within the Transitional Programs, you will support the daily operations & cleanliness of the Day Center & Resource Center at HOAP & HOST. You will provide drop-in support services for clients, collaborate with other team members to identify needs of clients, and provide backup to other staff as needed. Community Health Workers are open about their roles, responsibilities, and limits, and follow through on all agreed upon tasks.

SPECIFIC DUTIES:

  • Builds trust and rapport with clients through professional, respectful, caring, non-judgmental, culturally aware, clear, and honest communication, including listening carefully, gathering and sharing information, and resolving conflict.

  • Utilizes motivational interviewing skills to offer patient-centered, individualized resources that bridge barriers, connect clients to community resources and build skills to progressively move towards navigating the health care system on their own. Understanding of CHW role to “meet people where they are”, support clients to learn and use resources to improve their health and become advocates of their own care, & proactively participate in Care Coordination.

  • Uses Electronic Health Record platform to complete appropriate documentation of client and patient interactions, respond to referrals, and complete tasks in a timely manner.

  • Works efficiently within an interdisciplinary care team, including prioritizing and responding to warm handoffs, handling multiple priorities while remaining professional, strong interpersonal and communication skills, closed loop communication, contributes to teamwork, and serves as a liaison between the client and community resources.

    QUALIFICATIONS:

  • Minimum one year work, volunteer, or lived experience within a human service-related environment and/or bachelor’s or associate degree in public health, behavioral science, or social services, or closely related experience.

  • Ability to build effective working relationships, solve problems in a self-directed manner, and maintain a strong sense of professional boundaries.

  • Oregon Driver's License and proof of auto insurance (this position requires driving, and a motor vehicle report is conducted prior to hiring)

    PREFERRED QUALIFICATIONS:

  • Bilingual English/Spanish preferred

  • Completion of a Community Health Worker training course or certification is preferred. Non-Certified candidates must enroll in and successfully complete CHW Certification course after initial 90-day employment period.

  • Experience working directly with underserved and under resourced populations.
