Infinite Health Collaborative (iHealth) RN Navigator in Saint Paul, Minnesota

Description The RN Navigator provides support, advice, teaching, and counseling to patients and their families with colorectal cancer. The Navigator will work collaboratively as part of a multi-disciplinary team to provide patient and family education and will communicate with health care providers throughout the coordination and facilitation of services within the patient journey. Part time role, Monday - Friday 7 am - 5 pm, hours to vary, out of our Coon Rapids & St. Paul locations. Colon and Rectal Surgery Associates is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare. Essential Functions: Responsible for providing support, advice, teaching and counseling to patients and their families with colorectal cancer in designated office locations. Coordinates care for patients undergoing non operative management of rectal cancer throughout all CRSAL locations. Collaborates with physicians to devise and update development of policies for colorectal cancer services. Any and all other duties as assigned. Education and Experience Requirements: Graduate of an NLN accredited school of registered nursing. Current licensure as a RN in the State of MN 3 - 5-year previous RN navigation experience, preferably with colorectal specialty preferred. Current CPR certification required or completed upon hire. Essential Requirements: Ability to: Work in the clinic or hospital during business hours. Travel independently throughout the clinic/hospital (which may include movement from floor to floor); frequent bending, lifting, stooping or sitting for long periods of time may be required. Travel to other work locations as required. Work at an efficient and productive pace, handle interruptions appropriately and meet deadlines. Converse in a respectful and professional manner. Comply with company policies, procedures, practices and business ethics guidelines. Complete job required training. Comply with all applicable laws and regulations, (e.g. HIPAA, Stark, OSHA, employment laws, etc.) Demonstrate prompt and reliable attendance. Prioritize workload while being flexible to meet the expectations of the daily operations. Apply principles of logical thinking to define problems, establish facts, and draw valid conclusions. Understand and execute a variety of instructions. Effectively operate clinic and office (e.g., PC) equipment; and communicate on and operate the phone system.
