Region IV Area Agency On Aging Community Health Worker in Saint Joseph, Michigan

Position Overview: We are seeking a proactive and detail-oriented Clinical Quality and Compliance Associate to join our team. In this role, you will play a vital part in maintaining the quality and compliance standards of our agency's services. From tracking data to coordinating training programs, you will collaborate with various departments and community services/vendors to uphold our commitment to excellence.

About the Program: Integrated Care at  Home provides quality healthcare directly to individuals in their homes. Integrated Care at Home Services, in collaboration with the Area Agency on Aging, offers home-based medical care to adults aged 18 and older with disabilities or chronic health conditions in Berrien, Cass, and Van Buren counties. Our mission is to reduce emergency room visits and hospitalizations by delivering primary care, behavioral health counseling, chronic care management, and nutrition counseling where it's needed most -- at home. To qualify, individuals must be 18 years or older, primarily homebound, and managing two or more chronic conditions.

Qualifications:

  • Bachelor's degree preferred, or associate degree in Health or Human Services, or equivalent experience working with older adults/persons with disabilities.
  • Experience in working with older adults and/or persons with disabilities preferred (2-4 years).
  • Current Michigan license or certification (CHW, LPN, RN, LBSW, LLMSW, or LMSW) is desirable but not required.
  • Ability to obtain certification as a Community Health Worker within 1 year of employment (Training is paid for by the Agency)
  • Proficiency in standard office applications and computer literacy required (Microsoft 365 products).
  • General understanding of long-term care, person-centered thinking, and community resources preferred.
  • Ability to travel within Berrien, Cass, and Van Buren counties; reliable transportation required.

Benefits:

  • Full-time position (37.5 hours/week), no weekends or on-call duties.
  • 50/50 hybrid schedule after training period
  • Flexible scheduling between the hours of 6am and 7pm Monday- Friday
  • $200 continuing education/ per year
