Job Details Job Summary: The Cardiothoracic Advanced Practice Provider (APP) is a member of the medical care team whose primary responsibility is the management of complex and independent patient evaluation and management functions. The Cardiothoracic APP shall provide patient care commensurate with their level of advancement, competence and privileges, under the supervision of appropriately credentialed attending. The Cardiothoracic APPs require excellent interpersonal and communication skills integral to the success of a Cardiothoracic APP. The Cardiothoracic APP converses with patients in a manner that promotes understanding and develops positive clinician-patient relationships that helps to enhance good health outcomes. A Cardiothoracic APP must also have strong analytical skills to study clinical situations correctly and be compassionate to make the health of patients their priority and primary concern. The Cardiothoracic APP contributes to the collaborative patient care, to the research, teaching and consulting of patient care, provides leadership to St. Francis Hospital & Good Samaritan Hospital and to the PA & NP profession. Responsibilities Include: The Advanced Practice Provider will assist in collaboration with the physicians, in providing assessments, diagnosis, care and treatment of patients in an inpatient cardiology setting and will document findings in a thorough consultation / progress note. Review and implement admission orders and pre procedure orders / post procedure orders, participates in daily rounds and performs follow up tasks and acts as a liaison between the Cardiothoracic Surgeons, Nursing staff, unit based APPs and attendings Assist Cardiologist with noninvasive and invasive procedures and may perform procedures independently when assessed to have adequate skill Understanding of implanted device patients; pacemakers, defibrillators, and loop recorders. Appropriate ordering of diagnostic tests and medications Receiving, interpreting, and communicating internal and external testing results Coordinate pre-operative, post-operative, discharge and follow up care to patients through written and verbal communication Rounds daily on pre / post cardiothoracic procedural patients, examines and documents findings in daily progress notes in collaboration with the Cardiothoracic Surgeons Coordinating admissions and/or transfers to other facilities Consult and coordinate with multidisciplinary team in formulating an appropriate patient plan of care Serves as a liaison and resource to the Cardiac Cath Lab, staff, and physicians. Works with the Cardiac Quality Data Specialist to ensure ACC and NYS DOH reporting compliance is maintained. Provides support to the Data Specialist as needed when able to complete ACC / NYS DOH reporting forms Additional Responsibilities Include: Direct clinical services for medically ill adult patients according to the established SFH & GSH protocols and utilizes evidence based clinical practice standards under the supervision of an appropriately credentialed attending. Provide safe, effective and compassionate patient care. Devotes attention and resources to provide the best customer service possible. Address patient medical problems in the absence of the patient's attending of record. Examines patients, assess the patient's physical condition, identifying signs and symptoms and gathering personal details to arrive at a plan of care in collaboration with the attending of record. Participates in the medication reconciliation process on admission, discharge or when transfer of care to another provider or level of service. Collaborates with Cardiothoracic Surgeons on patient plan of care and coordination of documentation Documents patient plan of care by appropriate and prompt maintenance of medical records, orders, notes and timely signing of verbal and telephone orders. Explain the necessity, preparation, nature and anticipated effects of procedures to patients, patient's family, staff and healthcare students. Assess family coping skills and the need for crisis or other interventions. Practices proper infection control and sterile technique. Demonstrates ongoing improvement/proficiency in bedside invasive and noninvasive procedures and maintains patient procedure logs. Requirements: PA - Completion of an accredited Physician Assistant program, minimum Bachelor’s degree required NP – Current RN license and current AANP or ANCC certification Certification: BLS/ACLS/AED training required and accepted from only 2 organizations: American Heart Association and American Red Cross - BLS/ACLS for Healthcare Providers (CPR & AED) Program No online programs from any organization will be accepted.DEA registration required for those with discharge Salary Range USD $63.38 - USD $89.34 /Hr.
