Jewish Senior Home LWCC Speech Language Pathologist in Rochester, New York

Per Diem Pay Rate: $44.48

GENERAL SCOPE: The Speech-Language Pathologist will provide speech, language, cognitive and swallowing evaluations and treatments to residents referred by physicians for the purpose of diagnosis, restoring function, maintaining function, and maximizing functional skills. The Speech-Language Pathologist will function as part of the multidisciplinary team, providing education and instruction to staff, residents and families as needed.

EDUCATION / QUALIFICATION: Master’s degree in Speech-Language Pathology. New York State license in Speech-Language Pathology. ASHA certification is preferred.

ESSENTIAL FUNCTIONS:

  • Administer speech-language, cognitive and swallowing evaluations upon written order by a physician.

  • Provide speech-language, cognitive and swallowing therapy as indicated from the evaluation and with a physician’s order.

  • Establish a plan of care and integrate it into the interdisciplinary care plan.

  • Complete written evaluations, progress reports, discharge summaries and other program records as per policies and procedures and state and federal guidelines.

  • Participate in resident care conference as indicated.

  • Submit a record of evaluations and treatments along with a monthly statistical summary.

  • Inform the administrator of department, needs, strengths, weaknesses and problems and advise accordingly.

  • All other duties as assigned.

    EXPERIENCE: Completion of CFY. Experience working with the geriatric and/or adult population with neurological communication.
