Responsibilities

General Description

The social worker will provide psychosocial assessment and psychiatric diagnosis and treatment for older adult patients 65 years of age and older in the Strong Behavioral Health Older Adult Service. The social worker functions as primary therapist and is responsible for the overall coordination and implementation of the treatment plan for patients assigned to his/her caseload. The social worker participates on an interdisciplinary treatment team and develops liaisons with health and social agencies in the community, as well as with other health professionals within the hospital to support evaluation and treatment of the patient. The social worker also provides individual and group treatment, and crisis intervention as appropriate. The social worker may provide family treatment and case management services as indicated.

Specific Responsibilities

  • Provides social work treatment and direct clinical services to patients and families, including screening, evaluation, and group treatments. Performs psychosocial assessment, diagnostic formulation, crisis intervention for patients assigned and, when indicated, their families. This may be in the following modalities; individual, family and/or group therapy. Coordinates activities related to the comprehensive treatment and discharge planning for the patients assigned to his/her caseload.

  • Functions collaboratively and consultatively as a member of an interdisciplinary team. Ensures the development and implementation of comprehensive and individualized treatment plans for patients. Requests and coordinates psychiatric consultation when indicated.

  • Provides social work case management services as indicated. Coordinates with other resources in the community and within SMH to ensure continuity of care. Participates as a member of an interdisciplinary treatment team involving consultation, psychosocial diagnosis, treatment planning, and periodic care review.

  • Documents treatment and interventions in patient’s chart in compliance with Division, Department, and Hospital policy.

  • Participates in the development of a supervisory contract and in annual performance evaluation.

  • Attends Social Work Division and Program Staff Meetings.

  • Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice and patient care.

  • Participates in the routine committee process of the Social Work Division and complies with Social Work Division and Departmental quality assurance and staff development activities.

  • Participates in activities of the Social Work Division including, but not limited to, timely completion of time reporting and other Division forms; Hospital and Division mandatory in-service training programs, and UHS reviews.

  • Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competency events and training appropriate job duties.

  • Provides back up social work services as requested and able to other unit/programs during the absence of certain co-workers.

Other duties as assigned

Teaching

  • The social worker may provide supervision and training to social work students as determined by the social work supervisor in consultation with the Director of the Older Adults Service and the Clinical Manager for Social Work.

  • The social worker may participate in teaching concerned with issues of patient and family assessment and treatment interventions as they related to the psychosocial aspects of patient care.

Research

  • The social worker may participate in interdisciplinary and/or social work research endeavors as determined by the social work supervisor in consultation with the Director of the Older Adults Service and the Clinical Manager for Social Work.

Position Qualifications

  • Master's degree from an accredited school of social work (CSWE) required

Preferred:

  • Prior experience working in behavioral health clinical setting preferred.

  • Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions;

  • Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families;

  • Dedication to delivering a team-based integrated model of care

  • Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome.

Required:

Master's degree from an accredited school of social work. Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
