UR Medicine Home Care RN - Weekend Manager in Rochester, New York

Position Summary:

Responsible for the clinical, fiscal, and administrative management of the provision of service to patients who receive home care services on the weekends.

Responsibilities:

Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:

  • Directs and coordinates clinical operations across the weekend, including supervision of and participation in patient triage and scheduling of patient care for UR Medicine Home Care (URMHC) patients.

  • Assumes 24-hour accountability for the supervision of all professional visiting staff on the weekends. Ensures that professional field staff clinical competencies are completed as required by regulation and that staff adhere to agency standards. Initiates quality improvement plans with staff not meeting standards.

  • Administers annual performance evaluations per agency policy, coaching and mentoring staff per their individual needs.

  • Provides leadership to ensure appropriate multidisciplinary communication, collaborative work relationships and customer-oriented environment. Responsive to the needs of internal and external customers.

  • Participates in orientation of new professional home visiting field staff by assigning a preceptor, directly reviews documentation for accuracy and compliance, provides observation field visits, and coordinates skills competencies for weekend staff.

  • Responsible for the Key Performance Indicators (KPI’s) established by their manager.

  • Other duties as assigned.

    Qualifications: Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

  • Education:

  • Bachelor’s degree in nursing required.

  • Master’s degree in health services management or clinical education preferred.

  • Experience:

  • Home Health Care experience preferred.

  • Three (3) years of demonstrated effective leadership; organizational, team building and teaching skills, and ability to anticipate and proactively prevent problems.

Licensure, Certification, and/or Credential Requirements

  • Current RN licensure to practice in the state of New York.

Knowledge, Skill, & Ability Requirements

  • Demonstrated effective leadership, teaching, and communication skills.

  • Ability to perform nursing procedures effectively and safely.

  • Knowledge of regulatory and other accrediting agency requirements.

  • Outstanding interpersonal and communication skills both verbal and written.

  • Strong analytical skills to detect and analyze trends: clinical, financial, and programmatic.

  • Excellent working knowledge of regulatory standards and reimbursement issues in home care.

  • Ability to work with staff, patients, and families from diverse backgrounds.

  • Demonstrates ability to work with computers.

  • Microsoft Office Suite: Outlook, Word, Power Point and Excel

  • E-Record (electronic medical record systems)

  • Office equipment, such as computers, printers, copiers, and fax machines, as well as multi-line phone systems

  • Travel throughout service area year around

Salary Range: $59,384 to $87,214
