Trillium Health NY Registered Nurse, HIV Services in Rochester, New York

Purpose: To act as a liaison between clinical care team and case management to support the medical needs of people aging with HIV, providing direction regarding medical follow-up and facilitating linkage to case management services to support the patient/client.

  • Clinical Guidance and Coordination

  • Provide guidance related to clinical areas as needed.

  • Demonstrate leadership, good judgment and sound decision making when handling situations that require clinical intervention.

  • Work directly with clinical and Community Supportive Services teams to establish best practice and efficient systems for quality service delivery.

  • In collaboration with clinic leadership, provide support and input into establishing and maintaining Trillium Health’s certification as an Age-Friendly Health System through the Institute for Healthcare Improvement (IHI), both level 1 and level 2 recognition, and any other appropriate recognitions or certifications that may emerge.

  • Act as a liaison between Clinic and Supportive Services team, working within Patient Care Team model to identify patient medical and supportive service needs, making referrals to Care Management as appropriate and coordinating care with all care team members.

  • RN Responsibilities

  • Use a degree of personal, clinical and other professional judgment to solve problems and escalate to governing bodies as appropriate.

  • Internal and external collaboration. Utilization of a pro-active care management models to include:

  • Referrals to Services

  • Negotiation of Services

  • Coaching and skills development

  • Addressing personal, cultural, and systemic barriers to engagement in care

  • General education/risk reduction education inclusive of client assessment tools to assist in the guidance of healthcare improvement efforts that are client specific

  • Navigation of healthcare systems and ongoing support

  • Continual pursuit of age-appropriate care and activities

  • Increase patient’s active engagement in their healthcare

  • Increase positive holistic health outcomes of adults living with HIV

  • Reduce barriers improving Health Equity

  • Patient program retention

  • Other job duties as applicable

  • Complete documentation inclusive of the above activities and other applicable efforts to meet grant and funder standards for review by NYSDOH AI staff.

  • Works with Manager, Grant Funded Services to ensure timely connection to case management and supportive services.

  • Administer assessment tools to patients, where a licensure is required, and professional clinical guidance as needed.

Qualifications

  • RN/ BSN with minimum of two years of experience working in the field of HIV/AIDS or other chronic illness and a minimum of two years of management experience, with demonstration of collaboration.

  • Must maintain an active license as a registered nurse.

  • Experience with Geriatric and HIV population preferred.

  • Computer proficiency is required.

  • Fluency in Spanish and/or ASL desirable.

  • Must have a positive attitude, demonstrate initiative and professionalism.

  • Candidate must possess an understanding of the community level work and the ongoing importance of collaborating with outside organizations to meet the clients’ needs.

  • Must be accustomed to diversity.

  • Ability to maintain confidentiality is essential.

  • Experience in managing client case load assignments and functioning within a management team.

  • Must be able to work flexible hours including weekdays, evenings and weekends when needed. Must have a reliable vehicle, a valid NYS driver’s license, and proof of required automobile insurance.

Physical Requirements

While performing the duties of this job the employee is required to stand, sit, and walk. Occasionally the employee must stoop, bend and lift or move up to 25 lbs.
