Responsibilities
Position Summary:
The project nurse is responsible for data collection and assistance with transition of care and patient outcome initiatives within the Comprehensive Stroke Center. The QI Abstractor completes chart review and data abstraction for Get with the Guidelines (GWTG) and other stroke-related quality data as needed. GWTG is a software program designed by Quintiles partnered with the American Heart Association (AHA) to help hospitals measure guideline adherence and improve quality for acute stroke treatment and prevention. Assist with transitions of care and outcomes responsibilities through post-hospital patient telephone calls and chart review.
Responsibilities:
Review the chart of each stroke patient admitted to the hospital (includes ischemic stroke, intracranial hemorrhage, TIA, and subarachnoid hemorrhage).
Completes Get with the Guidelines (GWTG) form for each patient with a critical eye for potential quality issues and identify areas for improvement.
Assist with maintenance of stroke patient database at the discretion of Assistant Stroke Coordinator and Stroke Program Coordinator.
Perform post-discharge telephone follow up as part of transitions of care from inpatient to ambulatory setting.
Perform 90-day outcome assessment of select stroke patients by telephone and/or chart review to establish Modified Rankin score and other outcome measures at the direction of the Stroke Director and Program Coordinator.
Other responsibilities as assigned.
Qualifications:
RN preferred, LPN may be considered.
Health services coordination experience preferred.
Quality assurance experience and computer skills beneficial preferred.
Excellent communication skills and attention to detail are a must required.
