Pay Rate $50.00/hour

Per Diem

The physical therapist is responsible for overseeing the provision of physical therapy services and providing direct service to patients and residents and supervising the physical therapy assistant.

ESSENTIAL JOB FUNCTIONS

  • Reports to Director of Rehabilitation

  • Render physical therapy treatments only with a physician’s referral.

  • Provide/supervise treatment, revising as needed.

  • Recommend and assist in providing residents with functional equipment (wheelchairs, ambulatory, aids, etc.)

  • Work closely with physician, other professionals and consultants to integrate physical therapy into the multi-disciplinary team care plan.

  • Participate in patient/resident and family education and meetings, including team conferences, home evaluations and discharge planning.

  • Serve as member of skin integrity team and provide therapy-related treatment and recommendations as needed.

  • Document patient/resident admission and discharge evaluations per department policy.

  • Document progress notes to update status of those patients/residents receiving physical therapy per department policy.

  • Document readmissions, re-evaluation, consultations and other specific requests per department policy.

  • May provide supervision for no more than one physical therapy assistant.

  • May assist in arranging weekend coverage for physical therapists.

  • Track daily occupational therapy referrals to make sure they are being evaluated in a timely manner.

  • Record daily the number of treatment types provided and time spent per department policy.

  • Record daily the number of treatments provided and time spent per occupational therapy department policy.

  • Manage RUGS categories to ensure patients are placed in the appropriate categories necessary to optimize both reimbursement and time spent in therapy.

  • Assist director in maintaining oversight of daily therapy schedule in order to maximize utilization of staff.

  • Oversee provision of monthly statistics to fiscal services and administrator at the end of each month.

  • Assist in provision of in-service education, related to physical therapy, for rehabilitation personnel on an on-going basis.

  • Provide facility in-services as needed.

  • Assist in new staff orientation program as needed.

  • Attend departmental in-service, staff meetings and required facility in-services and meetings.

  • Provide acceptable dept. standard of productivity in providing therapy to residents.

  • Update skills and knowledge of occupational therapy issues/techniques related to the geriatric population.

Attend one or more professional presentations/conferences per year.

Requirements

QUALIFICATIONS:

  • Bachelor's degree plus a license to practice physical therapy

  • Two years’ full time paid experience in practicing physical therapy in a nursing home or health care facility.

  • Ability to communicate effectively, both written and verbal.

  • Good organizational skills. Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors and the general public.

Performing duties of this job requires occasional sitting, walking and standing. Must be able to occasionally lift loads of 30 pounds without assistance and the ability to sit, talk, and hear is required. SUPERVISORY RELATIONSHIP:The work is performed within established guidelines, and requires periodic supervision. The employee exercises some initiative and independent judgment to effectively perform the essential job duties of this position.

RESPONSIBILITY FOR OTHERS:The employee has no direct responsibility for others.

SAFETY RESPONSIBILITIES:Shall be fully acquainted with all safety policies and procedures of the Church Home. Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager. Must comply with facility in-service requirements.

Maintain confidentiality of all information related to the organization, residents, participants, family and staff, that may be encountered, either formally or informally, during the normal course of business. This includes medical and treatment records, financial and human resources information.
