University of Rochester Pharmacist,Pnd Lic,Out ofState - Pharmacist - Clinical Specialist (Toxicology) in Rochester, New York

GENERAL PURPOSE:

This is a temporary job classification, not to exceed 6 months, limited to pharmacists who have an active and current license in another jurisdiction (e.g. state) and are eligible for and pending licensure and registration in New York State. The out of state pharmacist pending licensure will not be permitted to practice as a pharmacist and will have a limited access profile in the electronic medical record. This person may complete training activities and work in a limited capacity, under the supervision of a licensed pharmacist. The individual may assist in dispensing and clinical activities if supervised by a licensed pharmacist.

JOB DUTIES AND RESPONSIBILITIES:

  1. Completion of training in relevant areas of the department under the supervision of a licensed pharmacist. Training may include activities related to the electronic medical record functionality, pharmacy operating systems, medication dispensing systems, sterile compounding, non-sterile compounding, general pharmacy operations, and provision of clinical pharmacy services.

  2. Documents clinical pharmacy activities as appropriate in the medical record or other systems (e.g. event reporting system), with cosignature by a licensed pharmacist.

  3. Works collaboratively with other clinicians and health-care providers to implement and maintain innovative disease or medication management programs and clinical pharmacy services, under the supervision of a licensed pharmacist.

  4. Participates in the development of global medication use management programs within the University of Rochester Medical Center including clinical guidelines, critical pathways, disease management and drug use programs, under the supervision of a licensed pharmacist.

  5. Conducts QA, MUE, or other research to document the value and contribution of pharmacy services at URMC. Participates in departmental Q/A processes such as reporting and following up on medication incidents, reconciling MARs, and cost-savings initiatives etc.

  6. Under the supervision of a licensed pharmacist, resolves problems with appropriate and direct communication to all pertinent staff in a clear and concise manner. Identifies patient care problems and does not pass on problems to the next shift without exhausting reasonable attempts at resolution and communicating progress. Contacts supervisor for assistance when solution is beyond his/her expertise or authority.

  7. Holds active membership and participates in local, regional and national professional organizations in areas of practice. Provision of continuing professional education to peers in the community, region, and/or nationally. Serves on departmental, hospital or medical center committees or task forces

QUALIFICATIONS:

Doctor of Pharmacy or BS Degree (pharmacy) required. Experience varies based on permanent position. BPS Board Certification in a relevant field of practice preferred. Active and current Pharmacist License from another jurisdiction, and eligible for New York State Pharmacy Licensure and Registration required.

Pay Range

Pay Range: $88,795 - $124,301 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
