Day Services
Responsibilities
Oversees the day to day operation and ongoing development of assigned Day Habilitation Services. Assists in assuring quality, person-centered services as outlined in each person's Individual Service Plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Assists in assuring the proper supervision, training and evaluation of all Day Hab staff; makes employment recommendations to the Coordinator of Community Integration Services.
Assists in the development, implementation, and monitoring of outcomes and activities as described in individual service plans related to Day Hab services.
Assists in developing and maintaining community volunteer opportunities and other community resources to ensure integrated, meaningful activities for all consumers.
Communicates and assists in the coordination of services with individuals, their families, and other service providers and/or supports.
Monitors the provision of all assigned habilitative and clinical services at home base and community sites and coordinates transportation services. Provides observations and feedback to the assigned staff on a regular basis.
Assures that all assigned services operate in compliance with the applicable standards and regulations and are consistent with Agency policies and mission.
Fulfills all required record keeping and documentation, including the development of a Day Hab plan for each individual receiving services.
Serves as a role model while promoting positive, professional behaviors and interactions for all staff and individuals receiving support. Maintains neat appearance, hygiene and grooming.
Communicates the needs of individuals and assists in obtaining needed equipment and supplies necessary to provide effective day habilitation services.
Maintains work schedule of all assigned staff and assures adequate coverage of all groups. Coordinates time off authorizations, verifies time sheets, and monitors PTO balance.
Performs all other duties relevant to the position as requested.
EDUCATION and/or EXPERIENCE: Bachelor's degree in a related field and one* year of experience in a setting serving individuals with disabilities, or Associate's degree in a related field and three* years of experience in a setting serving individuals with disabilities, or a High School diploma or equivalent and five* years' experience in a setting serving individuals with disabilities for which one year must have been in a supervisory capacity. *An exceptional candidate without the required experience may be considered. Must meet LAI's Vehicle Operator Requirements.

Commensurate with experience.
