JOB TITLE: Care Manager – Elder Justice

DEPARTMENT:Healthcare Initiatives

PROGRAM: Community Care Connections

SUPERVISOR: Director of Community Care Connections

STATUS: Non-exempt (Full-Time)

HOURLY Rate: $20.00 to $22.00 per hour

SUMMARY

In this innovative project, Care Managers will bridge the gap between Adult Protective Services (APS) and community-based aging services. Care Managers work with older adult APS clients referred to Lifespan who would benefit from programs and opportunities available within Lifespan and the community. This job posting is for a specialized Care Manager (CM) who will support engagement of APS-referred clients to maintain client health, safety and independent functioning in the community. The Care Manager helps clients secure safe, appropriate, and affordable housing, secure financial benefits and entitlements, link with in-home supports, link with appropriate social supports and activities and connect with needed medical care, providers, and transportation. Care Managers in this program are in the unique position of helping older adults when they are most vulnerable, at risk and in need.

DUTIES AND RESPONSIBILITIES

1.0 Provide care management to clients.

1.1 Accepts referrals as assigned.

1.2 Completes comprehensive in-home assessments to determine needs (with APS on joint cases).

1.3 Implements client-centered care plans that meet client needs and preferences; modifies service plans as required by changes in client circumstances.

1.4 Provides ongoing information and referral, advocacy, benefit and entitlement counseling.

1.5 Records case activity in the Lifespan database designated for the project.

2.0 Arranges mental health interventions for older adults and their caregivers, when necessary.

2.1 Conducts mental health screenings when appropriate.

2.2 Provides supportive counseling to older adults.

2.3 Identifies and provides linkage to appropriate ongoing mental health community services.

3.0 Participates as a member of the inter-agency team.

3.1 Participates in all Community Care Connections team meetings and trainings.

3.2 Participates in all Lifespan staff meetings and trainings.

3.3 Participates in quality assurance activities.

3.4 Provides input into the continued development of this innovative program.

3.5 Accepts and seeks guidance and clinical supervision.

4.0 Maintains up to date professional knowledge of the geriatric field, community services entitlement benefits, clinical skills and competencies through reading, workshops, and conferences.

4.1 Participates in training required for grant project activities.

5.0 Is aware of and actively supports the Agency’s initiative of being collaborative and community focused, innovative and culturally competent.

6.0 Other duties as assigned by supervisor.

Requirements

QUALIFICATIONS

EDUCATION: Bachelor or Master of Social Work or equivalent degree in fields related to health and human services.

EXPERIENCE:At least one year of experience coordinating community-based services for older adults.

PHYSICAL AND MENTAL REQUIREMENTS

· Must be able to meet with clients in their homes.

· A portion of the work is performed at a desk or at a personal computer station.

· Verbal, written and telephone communication skills are required.

· Strong written and computer skills needed.

· Interface with all levels of personnel.

· Ability to work with older adult clients at risk of abuse, neglect, exploitation and self-neglect.

· Ability to maintain composure under strict deadlines and difficult situations.

· Ability to work in multi-cultural situations.

Salary/Hourly Rate

Multiple factors will be considered for salary/hourly determination including, years of relevant service, skills required, education, and internal equity.
