Specific Responsibilities:

This is a Records Technician position in the Tuberculosis (TB) Program at the Northeast Health Center. This position is responsible for data entry and obtaining necessary information that supports primarily TB Program operational activities. Serves as the backup and support the STD Program. This position is responsible for reviewing, processing, and updating relevant clinical and demographic information. Supports the surveillance unit of the TB Program and works with others in an effort to accomplish the goals of the unit and the Health Department. Work is performed under the supervision of the Senior Human Services Program MGR - SES. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. Maintain state and local confidentiality/security policies as related to data and information in accordance with Florida Statute 392, Florida Administrative Code 64D-3 Health Department guidelines. Confidential Information sets include Social Security Data and clinical information. This position has responsibility to view and update confidential information.

Reviews each reported TB case or suspect of reporting delays, completeness of information or other issues. Ensures reports meet criteria, promptly provides the Tuberculosis Suspect/Case Reporting Forms to the Lead Nurse Case Manager (NCM) for prioritization and follow up. Immediately notifies the TB Program Manager and the TB Senior Human Services Manager of situations of a high profile, such as reports on residents of a nursing home, students or staff from schools, inmates from a jail, reports on children under the age of five, and other.

Responsible for registering all new reported episodes on TB Suspects and Cases into the TB Database. Responsible for registering clients in Health Medical System (HMS). Initiates care coordination, assigns initial classification, enters X-Rays, HIV and TB Screening test results and updates. Maintains reporting information in various tickler files for updating and surveillance purposes in a confidential and secure manner. Follows up on pending and unreported data and update data as database as needed. Review and conducts search on new lab reports.

Assists processing data during large contact investigations. Serves as the backup to registers / inputs client’s data including but not limited to EPI INFO, and other databases.

Serves as a backup for the biological scientist II surveillance staff. Receives and processes reports from Electronic Disease Notification (EDN) clients, classes A, B1, B2, B3. Reviews reports, prints records and keeps individual folders on CDC EDN A, B1, B2, B3 reports. Ensures registration of EDN clients in HMS. Schedules appointments for clients and enters progress notes in database. Contact clients to inform of the date/time of the appointments, mails certified letters with appointments slip and documents. Follow up with client’s appointments as needed. Sends client’s EDN records to the CHD Medical Records office for medical evaluation. Continue monitoring all EDN patients until submitted and completed in EDN system and EDN TB Data spreadsheet. Documents updates and outcomes progress notes, TB Data spreadsheet and EDN TB-Follow up worksheet.

Notified the TB Senior Human Services Manager of any Interjurisdictional and Migrant clinicians report of TB cases and suspects. Responsible for registering inter-jurisdictional TB suspects and cases clients in HMS. Initiates care coordination, assigns initial classification, enters X-Rays, HIV and TB Screening test results and updates. Follows up on pending and unreported data and update if available at the time of receiving inter-jurisdictional reports. Promptly provides the information to the Lead Nurse Case Manager (NCM) for prioritization and follow up.

Receives and reviews any referral from the Department of Corrections and brings the report of TB suspects and cases to the Senior Human Services Manager.

Serves as a backup for the biological scientist II surveillance staff. Registers, makes appointments, and follow up on transferred LTBI patients linking them to services at the LTBI clinics for continuation of care.

Serves as a backup and performs STD Surveillance, and notification inclusive of reviewing STD Laboratories reports, calling providers to ensure treatment, and referral of clients to STD Field Staff by utilizing the PRISM System.

Attends training courses, staff and team meetings for the TB Program

Performs other duties as assigned.
