Licensed Practical Nurse (LPN) - Patient Centered Home Coordinator - Physician Office - Work at Home VA Richmond Market

JOB SUMMARY:

The PCMH coordinator is a member of the PCMH team who works within the Bon Secours Virginia Medical Group in the management of preventive care and chronic disease using the principles of the patient centered medical home, as outlined by the NCQA. The PCMH coordinator is an integral part of the PCMH team in the role of planning, assessing and designing workflow solutions and strategies for implementation and sustainment of the PCMH recognition for primary care and specialty practices.

ESSENTIAL FUNCTIONS:

Able to function independently and as a team in the planning, assessment, design, and implementation phases of NCQA PCMH recognition and other population health initiatives as needed. Guides individual practices to align their internal work flows and create an individualized action plan for adherence to the principles of PCMH. Ability to deal in a calm manner with multiple concerns and problems of clinical and non-clinical staff and to communicate those concerns to the appropriate leadership. Participates in the preparation and presentation of PCMH materials and strategies to the individual practice stakeholders. Facilitates the development of practice-specific plans for transformation, submission, and sustainability of PCMH 2017 standards for each individual primary care practice. Works to maintain timeline for completing practice goals, working toward submission of application. This includes: training of rooming protocols to LPN and Medical Assistant staff support of the Super User in training others in the practice the proper documentation of required components in ConnectCare training of Point of Care testing protocols as described in the Point of Care testing policy. Prepares and submits NCQA applications to include:

  • Collation of required reports

  • Chart audits to ensure proper documentation of Care Management and TOC

  • Capture of screenshots to show practice workflows

  • Communication with NCQA staff around practice changes and provider listings

  • Tracks NCQA application addressing any issues that arise and keeping leadership abreast of application status.

  • Participates in all PCMH team meetings and strategy sessions for a successful PCMH submission.

  • Reports to Project Manager, PI&I Manager, PCMH Manager, and other leaders the status of practice transformation and expected timeliness of submission.

  • Maintains current knowledge of PCMH 2017 Standards.

  • Attends NCQA webinars and updates, Super User calls, and EMR Advisory Council meetings in an effort to supply primary care practices with up to date information.

  • Works with PM, coordinators, analytic and support staff with regard to tracking the status of the action plan and leads team members in delivering timely materials.

  • Identifies and communicates the barriers in each individual practice to obtain the successful submission and helps to manage the development of solutions that are tangible, practical, and sustainable

  • Initiates the development of reports and policy catalog for purpose of a multisite application and maintenance of recognition.

  • Establishes strong interpersonal and facilitation skills with individual practice staff.

EMPLOYMENT QUALIFICATIONS:

  • The PCMH Coordinator must have an active license with the Commonwealth of VA as a Licensed Practical Nurse or have equal or higher levels of clinical education.

  • Experience in a primary care practice preferred.

  • CPR certification must be achieved and maintained.

  • Must be proficient in Microsoft Office – Word, PowerPoint and Excel required.

  • Strong Epic documentations skills required.

  • Epic Ambulatory certification preferred.

  • Experience in Athena scheduling and reporting preferred.

  • Must possess basic analytical skills for data analysis and presentation.

REQUIRED QUALIFICATIONS:

  • High School/GED (required)

  • Vocational/ Technical Degree or 2 Year/ Associates Degree

  • Licensed Practical Nurse or Medical Assisting

  • Active Medical Assisting certification (required): Certified Medical Assistant (American Association of Medical Assisting), or

  • Registered Medical Assistant (American Medical Technologists), or

  • Certified Clinical Medical Assistant (National Healthcareer Association, or

  • National Certified Medical Assistant (National Center for Competency Testing) or

  • Nationally Registered Certified Medical Assistant (NCRMA) (National Association for Health Professionals.

  • BLS Basic Life Support (preferred upon hire, required within 90 days of hire)

If incumbent is unable to obtain MA certification, an Assessment-Based Recognition in Order Entry (ABR-OE) is acceptable.

PREFERRED QUALIFICATIONS:

  • Prior experience within Physician Office

  • 2 years of practical nursing or medical assisting experience within a healthcare setting, preferably within a physician practice.

  • Medical Terminology, ability to use standard office equipment. Electronic Medical Record (EMR) experience.

  • Strong oral and written communication skills, and an ability to collaboratively work with patients, families and teams within a high-volume environment. Strong attention to details and an ability to multitask without prior instruction or direction. Demonstrated conflict resolution skills, and an ability to set priorities and lead teams.

SKILLS

  • Record patients' medical history, vital statistics, or information such as test results in medical records.

  • Interview patients to obtain medical information and measure their vital signs, weight, and height.

  • Prepare and administer medications as directed by a physician.

  • Collect blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing.

  • Provide authorized prescription and drug refill information for pharmacies as directed by provider.

  • Explain treatment procedures, medications, diets, or physicians' instructions to patients.

  • Clean and sterilize instruments and dispose of contaminated supplies.

  • Perform routine laboratory tests and sample analyses.

  • Perform general office duties, such as answering telephones, taking dictation, or completing insurance forms.

  • Successfully completes skills or competency checklists to be able to help physicians examine and treat patients, performing such tasks as handing them instruments, giving injections, removing sutures, changing dressings on wounds.

  • Strong oral and written communication skills

  • Ability to collaboratively work with patients, families, and teams within a high-volume environment.
