Responsibilities: - Provide direct patient care in a home health setting - Conduct comprehensive assessments of patients' health status - Develop and implement individualized care plans - Administer medications and treatments as prescribed - Monitor and evaluate patients' progress and adjust care plans accordingly - Educate patients and their families on managing their conditions and promoting overall health and wellness - Collaborate with other healthcare professionals to ensure coordinated and effective care - Document patient information accurately and timely

Experience: - Minimum of 2 years of experience as a Registered Nurse, preferably in home health or a related field - Strong knowledge of physiology and medical terminology - Proficient in performing clinical procedures such as gastric lavage, airway management, and laboratory specimen collection - Experience in providing care for patients with various conditions, including dementia care and primary care management - Familiarity with ICD-9 coding system for accurate documentation and billing purposes - Ability to lift and move patients safely

Skills: - Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare team members - Strong critical thinking and problem-solving abilities to make sound clinical judgments - Detail-oriented with exceptional organizational skills to manage multiple patients' needs effectively - Ability to provide health coaching to promote self-management and adherence to treatment plans

Job Type: Full-time

Salary: $55,000.00 - $87,018.00 per year
