The Community At Brookmeade Social Work Assistant in Rhinebeck, New York
Meadows at Brookmeade located in Rhinebeck, NY, is seeking a compassionate and motivated Social Work Assistant to join our dedicated team. This position will report to the Director of Social Work. You will assist in admits, discharges, care planning, conducting assessments, and coordinate services to enhance the well-being and quality of life of our residents.

Responsibilities:
· Review referrals for admission
· Review insurance and verify benefits
· Meet with families and conduct facility tours
· Build relationships with hospitals, social workers, and discharge planners
· Visit patients in hospitals and interact with social workers and discharge planners
· Maintain an accurate facility census and knowledge of available beds
· Help residents and family members to complete paperwork as needed
· Assist Marketing and Admissions Director with tasks as needed
· Assists with discharge and admissions planning
· Clerical duties
Qualifications:
· Associate’s degree in human services or other related field preferred (or equivalent experience)
· Previous experience in admissions or client services preferred
· Strong communication and interpersonal skills
· Ability to work collaboratively in a team environment
· Ability to prioritize and execute quickly in a fast-paced environment
· Proficient in documentation and record-keeping practices
· Compassionate and customer-focused approach
