Description THE ROLE Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.). The goals of Case Management include the achievement of optimal health, access to services, advocacy, appropriate utilization of resources and collaboration with post-acute care providers to ensure patient's needs are met in the community. The System Director of Care Management Education serves all Division Care Management Educators (Northern Division, Southern Division, Central Division ).The System Care Management Educator is responsible for oversight of all aspects of Systems standard education and training (care coordination and utilization management), for developing/maintaining standard workflows, on-boarding orientation, competencies, and policies; serving on The System Case management Leadership Committee and Chairing the System Educator Meeting, as a subject matter expert for care management. The Care Management Educator also aligns all educational actives with the established system wide process and oversees an annual review for all systems level education materials. ESSENTIAL FUNCTIONS Collaborates with professional colleagues, the Providence Nursing Institute, and various division Educators to develop, coordinate, implement, and evaluate clinical education, orientation, in-service, and continuing education for patient-facing caregivers throughout the Providence Healthcare System . Attends monthly Division education meetings and participates in collaboratives. Education & Quality Care: Manages system wide care management core curriculum to ensure caregiver core competencies (Care Coordination and Utilization Management). Facilitates minimum standards for on-boarding education and orientation for hires. Creates and Updates system standard workflows. Reviews Divisional Educational content and identifies best practices applicable to systems expansion. Collaborates and develops content in partnership with Divisional educators. Conducts training on CMS updates in partnership with Compliance. Evaluates training effectiveness and responds as appropriate Program Planning and Implementation: Assists in the program development, implementation and evaluation of new system wide programs. Active participant and attendance at all meetings as requested to include staff meetings, Providence meetings, and conferences. Acts as clinical leader in implementing strategic system level standards and initiatives . Staff Development: Serve as a mentor to Division educators for quality and continual process improvement. Develop and implements training programs for care management on best practices. Lead in project development to include but not limited to ; protocols, education practice guidelines, and other guidelines as appropriate. Develops competencies and collaborates with Division Care Management Directors/Division Educators to ensure timely completion of competencies. Supports Division Care Management Educators in providing training and working with Ministries who may need additional focused support. Knowledge: Review regulations, literature, professional journals, and attend conferences and program to update skills and continuously improve and maintain professional competence.
