Modivcare Facility Outreach Manager in Remote, Florida

This position is responsible for acting as an advocate within the non-emergency medical transportation system for patients and case managers by providing in-service training to facilities on how to use NEMT services and to promote relationships. In addition will partner with nursing homes, dialysis clinics, and other medical providers that have numerous clients with scheduled trips to maximize scheduling coordination and information sharing.

This position…

  • Advocates within the non-emergency medical transportation system for patients and case-managers.

  • Advocates on behalf of the organization's goals and initiatives within the community.

  • Provides in-service training to facilities on how to use NEMT services and to promote relationships.

  • Ensures facility outreach and training are conducted throughout assigned area of responsibility.

  • Coordinates all community-based outreach, event planning and education.

  • Develops and implements outreach strategies.

  • Develops on going relationship with facilities through face-to-face meetings, phone calls and group presentations.

  • Analyzes subscription service requests (standing orders), providing education and outreach to avoid or resolve problems and increase quality.

  • Analyzes patient attendance information for prior month from facilities.

  • Analyzes data and develops cost management strategies.

  • Oversees all facility coordination and quality assurance activities in partnership with the Director of Operations to include staff management.

  • Works to provide stakeholder engagement.

  • Identifies and contracts with Sole Source TPs; responsible for the successful rate negotiation and end to end sole source contracting function.

  • Leads facility networking, tracking and reporting outreach efforts.

  • Collects and analyzes feedback from the community and stakeholders.

  • Provides training and development of the TripCare portal.

  • Provides support and guidance to more junior team members

  • Participate in other projects or duties as assigned.

  • Business travel of up to 80% required.

We are looking for someone with…

  • Bachelor's Degree required.

  • Five (5) plus years member service, transportation service, UR Manager in a health plan or social worker experience.

  • Medical transportation management experience preferred.

  • Facility based social service programs, including facility discharge planning experience preferred.

  • Clinical, case management and discharge experience preferred.

  • Or equivalent combination of education and/or experience.

Salary: $60,800 - $82,100 annually
