Description Position Summary/Essential Duties: The PCNA provides personalized care to patients in support of their healing, wellness and overall experience as delegated by the RN/LPN. Duties include: * Assists patient with daily hygiene and grooming as needed * Assists patients with their daily meals as needed * Ensures patients are out of bed and in the chair for meals. * Provides oral care for patient specific needs * Ensures linens are changed and clean * Assists patients with basic necessities (pantry items and other hospitality needs) * Assists as needed with patient mobility, repositioning needs, and other activities of daily living * Assist with obtaining vital signs as needed * Documents in the electronic medical record Equal Opportunity Employer -- minorities/females/veterans/individuals with disabilities/sexual orientation/gender identity
