The Grand Rehabilitation & Nursing Nursing Home Social Worker in Queens, New York
Bridge View Nursing Home, part of The Grand Healthcare System is seeking a Full-time Staff Social Worker to join our team!
Experience in long-term care is preferred!
Duties Include:
Interviewing participants and their families
Provide psychosocial assessment and counseling to participant and families
Developing specific care plans based on the needs of individual patients
Helping participant apply for appropriate social services
Providing crisis intervention as needed
Providing grief and loss counseling and supportive counseling
Strengthening communications among participant, families, and the program or facility staff
Required:
- Bachelor's Degree
Preferred:
Long-Term Care Social Work experience
NYS Screen Certification
Strong Admissions and Discharge Planning experience
We offer the Following:
Salary $55,000.00 - $70,000.00 per year
