We are seeking a dedicated Social Services / Case Manager to join our team. The candidate will be responsible for overseeing and coordinating services for skilled and long term residents. This role requires strong program management skills and the ability to work collaboratively with clients and their support networks.
Duties:
- Coordinate care working with the families, Insurance case managers, and facility staff.
- Post Admission - Develop, implement, and review resident care.
- Utilization-Review and Care Plan meeting coordination
- Communicate with Family
- Issue NOMNCs
- Discharge planning - Attaining physician orders, DME, Home Health, transportation, and follow-up appointments
- Recruit other resident candidates through Case Managers
- Assist residents with Social Services needs
