Position Summary:

Responsible for the day to day clinical operations of the hospice program; Plans, coordinates, manages, and evaluates the performance of an Interdisciplinary team to ensure the delivery of quality patient care and provision of an excellent patient experience; Functions in a leadership, mentorship and resource role for the hospice team, ensuring compliance with TJC, CMS, BSMH, State and Federal regulations

Essential Functions:

Accountable for clinical operations of the hospice program in compliance with all applicable hospice regulations, accreditation standards, policies and standards of practice.

Coordinates new hospice referrals and admissions. Assigns to the appropriate hospice team.

Monitors staff assignments, caseloads, and productivity, promoting quality patient care and clinical outcomes

Facilitates the delivery of hospice services by acting as a resource for staff, physicians, referral sources, patients and other internal and external customers.

Coordinates Interdisciplinary Group Meetings (IDG) with the hospice team and promotes communication to ensure quality of care and coordination of services.

Participates in the hospice Quality Assessment and Performance Improvement (QAPI) Program, designed to improve quality of patient care, clinical outcomes and staff performance.

Demonstrates sound clinical judgement, decision-making, and clinical guidance in leading interdisciplinary hospice team. Monitors clinical processes and documentation to ensure all services are provided efficiently and effectively with appropriate resource utilizations.

Participates in the recruitment, interviewing, selection, onboarding, competency evaluation, retention and educational activities for the hospice team.

Audits medical records to ensure compliance with regulation, content, and accuracy.

Monitors certification and recertification compliance; ensures orders for all services and performs HIS data collection and submissions. Monitors compliance DNB’s through Connect Care.

Job Requirements :

  1. Graduate of an accredited school of nursing. BSN Degree preferred.

  2. Current licensure to the practice as a Registered Nurse in the Commonwealth of Virginia.

  3. Minimum of 3 years of hospice care experience. At least two years supervisory experience in a health care setting, preferably in hospice or home health

  4. Current CPR certification

  5. Valid Virginia Driver License and an acceptable driving record
