Kaiser Permanente Supervisor, Home Health in Portland, Oregon

Job Summary:

Assure the provision of high quality, reputable, and cost-effective clinical services meeting regulatory requirements through supervision and management of multidisciplinary clinical and support staff (licensed and non-licensed).

Essential Responsibilities:

  • Manages the delivery of quality clinical services consistent with agency policy, applicable state practice acts and applicable state and federal regulations through coordination of patient and staff schedules, provision of advice to staff and patients and directing utilization of program resources (clinical and non-labor).

  • Hires, orients, trains, develops and supervises qualified and competent Home Health staff.

  • Provides training and development, performance coaching, on-time yearly performance review, and corrective action to assure effective delivery of quality clinical services that consistently meets regulatory standards.

  • Provides performance oversight through clinical record reviews, home visits, team meetings, care conferences, investigation of patient complaints and adverse events.

  • Assures the delivery of cost-effective care through budget development and oversight, process monitoring and improvement, individual staff performance and effective utilization of non-labor resources (outside referrals, supplies, DME, etc.).

  • Plans, organizes, directs and evaluates clinical service delivery to continuously improve the quality and coordination of clinical care provided to patients and families.

  • Develops, reviews and implements policies, procedures and clinical protocols to assure the provision of state of the art hospice and palliative care.

  • Establishes, maintains and utilizes effective working relationships with peers, direct reports, managers, labor and physician partners to effectively improve the delivery of quality, efficient and compliant clinical services.

  • Assures ongoing professional development for self and staff.

  • Assures home health, hospice and home infusion patients and families, at home and in facility settings, receive high quality care through organizing and supervising clinical staff and providing after-hours administrative call.

Basic Qualifications:

Experience

  • Minimum two (2) years of field experience managing complex cases within a multidisciplinary setting in a home health or hospice department.

Education

  • Bachelors degree in nursing from accredited school, OR four (4) years of home health or hospice supervisory experience.

  • High School Diploma or General Education Development (GED) required.

License, Certification, Registration

  • This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire.

  • Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire

  • Registered Nurse License (Oregon) within 6 months of hire

  • Drivers License (in location where applicable)

  • Basic Life Support

Additional Requirements:

  • Basic knowledge of principles of supervision.

  • Working knowledge of Medicare certification, Oregon and Washington licensure, and The Joint Commission accreditation applicable to Home Health and Hospice programs.

  • Knowledge of nurse practice laws.

  • Working knowledge of the delivery of nursing services in the home setting.

  • Ability to operate computerized clinical documentation systems including use of a laptop/notebook computer.

  • Demonstrates effective interpersonal communication and conflict resolution skills.

  • Ability to prioritize patient needs to achieve attainable treatment goals.

  • Ability to facilitate effective working relationships between diverse groups and individuals.

  • Well developed problem solving and critical thinking skills with clinical and organizational relevance.

  • Well developed organization and time management skills.

  • Must provide dependable automobile transportation using safe driving to visit patient homes and attend off-site meetings.

Preferred Qualifications:

  • Minimum two (2) years of home health/hospice supervisory experience.

  • Knowledge of Medicare regulations and requirements.

  • Knowledge of care of the chronically ill and frail elderly.

  • Knowledge of palliative care of terminally ill patients.

  • Working knowledge of managerial theory.

  • Excellent time management, communication (written and verbal) and customer service skills.

  • Ability to utilize laptop and work station computer.

  • Keyboard by touch.

  • Working knowledge of the managed care industry.
