Reports to: Permanent Supportive Housing Services Manager

Location: SE Portland

Compensation: $24.00 - $26.00 hourly

    Who You Are:

    The PSH Case Manager will use proactive prevention, best-practice approaches, and collaborative interventions to support residents to thrive and retain their housing. The position works closely and coordinates with onsite Property Management staff, other Resident Services staff, and with external partnerships.

    What You Will Work On:

  • Provide evidence-based case management services to approximately 20 adults residing at Beacon.

  • Complete initial intake and assessments of to assess their needs in order to implement or connect them with services or treatment.

  • Use Housing First and harm reduction interventions and methodologies to provide resident centered services that support them in retaining their housing.

  • Assist residents in applying for and obtaining long term stable sources of income including but not limited to mainstream benefits, as well as OHP, SNAP, and other programs for which they are eligible.

    What You Bring:

    Education, Certifications, and Licensure Requirements/Preferences

  • Bachelor's degree in human services or equivalent lived and/or worked experience required.

  • Master's Degree in Social Work or in a related field preferred.

    Experience Requirements/Preferences

  • 1 year case management experience required.

  • 3 years of previous experience working with diverse populations and families in crisis preferred.

  • 3 years of case management experience within social services, homeless services, and/or a PSH setting preferred.

  • Experience working with and demonstrated understanding of unique challenges for vulnerable adults including those experiencing homelessness, trauma, mental illness, addictions, and chronic medical needs.

  • Ability to use Microsoft Office programs including Word, Excel, Outlook, and PowerPo int.
