REQUIREMENTS:
Bachelor’s Degree in Human Services, Medical, or related field + 1 year of experience OR High School Diploma/Equivalent + 5 years of related experience in the medical field and/or with persons experiencing behavioral health challenges.
Valid Driver’s license
Ability to drive an organizational vehicle
Act 33/34/73 clearances
Pre-employment drug screen
SCHEDULE
Generally Monday-Friday 9:00am-5:00pm
This is a non-exempt (hourly) position which may require some evenings and weekends to meet the needs of the persons served.

ABOUT THE ROLE: The Community Health Worker (CHW) will have a comprehensive understanding of the community supports and resources available to individuals with diverse backgrounds and behavioral health needs. The CHW builds a close trusting relationship to the persons served and establishes linkages to support services and resources. The effectiveness of the CHW lies in their ability to gain access to hard to reach, difficult to engage underserved individuals. The CHW partners with the person served to ensure they have adequate access to goals developed that address Social Determinants of Health (SDOH). This position will provide social support and advocacy for persons served. This position will also include increasing the visibility of Pittsburgh Mercy through community outreach. This community-based position will require home visits.
