Job Title: Registered Nurse (RN) -- Home Care
Job Type: Full Time
Work Location: Philadelphia County (In-Person)
Join our team as a Registered Nurse (RN) and provide top-tier home care services. Leverage your nursing skills to assess patient needs, develop care plans, and administer treatments. Collaborate with physicians and community health professionals to deliver comprehensive care.
Responsibilities:
- Completes an initial assessment of patient and family to determine home care needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Participates in on-call duties as defined by the on-call policy.
- Instructs, supervises and evaluates home health aide care provided every two (2) weeks.

Qualifications:
- Graduate of an accredited nursing school
- Licensed RN in the state of PA
- Valid driver's license and reliable transportation
- 1-2 years of acute care experience in an institutional setting
- Current state licensure and CPR certification
- Bachelor's degree and home health care experience preferred
